System Navigator Program (SNP)

The System Navigator Program (SNP) provides one-on-one services to individuals being released from Toronto South Detention Centre (TSDC). The program works with individuals to identify needs and provide navigation support and guidance through connection to community-based services to support their reintegration. The Systems Navigator staff provide intensive case management starting first with contact during pre-release (if possible), then supporting the client at release and during the critical few weeks after their release from the institution. Following a period of about 4-6 weeks, the SNP staff will carry out a “warm transition” of the client to social service agencies for ongoing care. Some of the services and supports that clients will be connected to through the SNP include, but are not limited to:

  • Income/Money Management
  • Legal

  • Social Supports

  • Accompaniment to service provider offices and appointments
