Systems Navigation

The Systems Navigation Program is a voluntary, goal-driven, case management program for men & women who have been recently released from a provincial correctional centre and/or are being supervised on probation or parole in Kawartha Lakes & Peterborough. Referrals must be made by institutional staff (e.g., Community Reintegration Officers, Social Workers, Discharge Planners, etc.) for those currently incarcerated, or by Probation and Parole Officers in Kawartha Lakes & Peterborough. We are not able to accept self or community agency referrals. The Systems Navigation team works with clients one-on-one to establish an individual intervention plan outlining their needs & the steps necessary to meet their desired outcomes. The Systems Navigation Program can support clients with any goals, including but not limited to:

  • Obtaining identification
  • Acquiring basic necessities including clothing & hygiene supplies
  • Securing income through Ontario Works (OW) or Ontario Disability Support Program (ODSP)
  • Accessing shelter or searching for housing
  • Food supports & assistance with food bank pick-ups
  • Accessing healthcare or scheduling medical & dental appointments
  • Connecting to community mental health or addictions supports
  • Support in attending appointments (e.g., Probation, Court, Fourcast, CMHA, etc.)
  • Income tax assistance
  • Social supports
