Supportive Housing Program

This program provides assistance in obtaining housing and intensive case management services to chronically homeless individuals/families referred from local shelters, jails, and hospitals and with individuals who are at high risk of homelessness. We support individuals in their own environment to foster and develop skills necessary for long-term self-efficacy and housing stability success. The program focuses on housing maintenance for those with significant barriers to housing stability, as well as social and community integration including connections to employment and education. This program is funded by United Way Sarnia-Lambton and the Ministry of Solicitor General.
