System Navigator

The Systems Navigator is responsible for providing case management to support community reintegration for individuals recently released from a correctional facility. The Systems Navigator helps individuals connect with appropriate services, provides guidance and maintains regular contact to ensure the individuals are supported and connected as they reintegrate into their community post-release.
