Reintegration Services

The Community Reintegration Worker is able to facilitate access to specialized community based programs and prevention services that facilitate healthy and positive reintegration in to the community. This is applicable for those individuals who are incarcerated and anticipating release, and vulnerable individuals in the community who are homeless, at risk of becoming homeless, and or who may require additional and unique community supports. After completing a brief assessment to determine ones’ acuity, needs and establish goals, the worker can connect individuals to various resources including the agency’s current programs and services. The program has established designated drop in days and times at the Barrie and Orillia office locations on Tuesdays and Wednesdays from 9:00 to 1:00PM. The worker has also partnered with various community agencies to deliver drop in services where the client need is the highest. The program addresses barriers that may include: access to housing, ID replacement, healthcare and doctor referrals, case management and counselling services, education and employment direction, and financial literacy, including access to Ontario Works and Disability Supports. The worker often starts the process providing assistance to those incarcerated, and in connection with the institutional worker develops discharge plans and community supports directly upon release. Currently, the service id offered in Barrie and Orillia, with expansion to Midland and outlying areas.
